Help! Health history pot use pattern questions

I am currently applying for my permit. In the medical history form it has a whole page devoted to marijuana use patterns. Specifically it asks first age used, was it social, how much do i use, and would I use more if cheaper and easier to obtain. What is the purpose of these questions? Is it for their information only or are there certain things they watch for that would disqualify someone? Thanks for the help =3
